Working together to prevent homelessness – Delayed Transfer of Care

Did you know that 40% of patients currently receiving care in mental health hospitals don’t actually need to be there?

Partnering with Greater Manchester Mental Health Trust (GMMH), we have been working on a project to support people living in mental health supported temporary accommodation to move into their own homes. The Delayed Transfer of Care scheme has helped people move into a home that that best meets their needs.

This includes individuals like Dave, who was in and out of mental health hospitals for eight years.
