Working together to reduce delayed transfers of care

Moving on from hospital or temporary accommodation should be a positive step. But without the right support in place, it can feel uncertain and difficult.

Our Delayed Transfer of Care service supports people who are ready to leave hospital and move into a home of their own, working with health and local partners to make sure the right support is in place from the start.

The focus is not just on moving home, but on helping people feel settled, supported and ready to take the next steps in their recovery.

What we’re aiming to achieve

We want people to:

  • Move on safely and at the right time, without unnecessary delays
  • Have the right support in place from day one in their new home
  • Feel settled, confident and able to live independently
  • Experience a joined-up approach across housing, health and support services

What’s changed

We’ve strengthened how we work alongside NHS mental health services and local organisations, helping people receive more coordinated support and avoid delays when moving into their new home.

This means people can access the support they need sooner, helping them move on more smoothly, settle more quickly and feel more confident during the transition.

  • Reduce delays in people moving on from hospital or temporary accommodation
  • Make sure support is in place early, helping people settle more quickly
  • Reduce uncertainty during what can be a significant life change
  • Support people to rebuild confidence and maintain their independence
  • Provide a more joined-up and consistent experience across services

Over the past year, we’ve supported 13 people to move into longer-term homes and delivered 574 support sessions, helping people access practical and wellbeing support tailored to their individual needs.

A big part of this work is supporting people through key moments in their recovery.

By working closely with partners, we’re making sure people don’t just move on, they do so with the right support around them, helping them feel more secure, build confidence and prepare for what comes next.

We know there’s more to do, so we’ll continue working closely with partners, listening to feedback and building on what’s working well, so more people can move on safely, maintain their independence and thrive in their communities.
