Supporting people to live independently through our Supported Housing service

Living independently isn’t always straightforward. At different points in life, people may need additional support to feel stable, confident and able to manage day-to-day life.

Our Supported Housing service works with local authorities, health services and support organisations to make sure tenants can maintain their home, with the right support available when they need it.

• Support people to sustain their tenancy and stay in their home

• Prevent situations from escalating by putting support in place earlier

• Help tenants feel more confident managing their day-to-day lives

• Improve wellbeing by reducing uncertainty and providing consistent support

• Create a more joined-up experience across housing, health and support services

Over the past year, we’ve delivered 6,901 support sessions, followed up 1,622 wellbeing alerts and carried out annual reviews with 94.9% of tenants. Together, these activities help people access support early, manage challenges and maintain their independence.

This approach helps people feel more in control of their situation and able to take positive steps forward.

Making a difference

A key part of this work is supporting people through challenges and helping them maintain stability.

This might involve helping someone access the right services, supporting them to manage their tenancy or building confidence to achieve personal goals.

One tenant shared: “The scheme coordinator is brilliant, they can’t do enough to help with information, advice and support.” This reflects the difference that consistent, personalised support can make.

There is a lot to be proud of across our Supported Housing service.

We will continue listening, strengthening our partnerships and building on what’s working well, so more tenants can feel secure in their home, maintain their independence and achieve positive outcomes.
