Living independently doesn’t always mean living without support. As people’s needs change, having the right help at the right time can make a real difference to how safe, connected and confident someone feels day to day.
Our Extra Care service works alongside care providers, health teams and local partners to support tenants to continue living in their own home, with the right support around them.
We want tenants to:
1
Feel secure and confident in their home
2
Stay independent for as long as possible
3
Feel connected to others and less isolated
4
Maintain their wellbeing and confidence
5
Experience support that is joined up and responsive to their needs
We’ve strengthened how we work with partners across our Extra Care schemes, so support is better coordinated and more consistent.
This means we can understand people’s needs earlier and make sure the right support is in place when it’s needed.
By working in this way, we are helping to:
- Support people to live independently in their own home for longer
- Improve wellbeing through consistent, personalised support
- Reduce isolation by helping people stay connected and involved
- Build confidence so people feel more in control of their day-to-day lives
- Provide a more joined-up and seamless experience across care, health and housing services
Over the past year, we’ve delivered 421 activities designed to support wellbeing and social connection, completed 364 weekly scheme inspections and ensured every tenant received an annual review of their support needs. Together, these measures help people remain safe, connected and independent.
This approach helps ensure people feel supported as their needs change, so they can continue living well in a place they call home.
A key part of this work is helping people feel that they are not facing things on their own.
By working closely with partners and being present within schemes, we can support tenants in ways that build confidence, encourage involvement, and help people feel part of a community.
One tenant shared: “I love living here. The onsite staff help with everything I need, and I feel safe and supported. Living here has helped me build my confidence and get involved in activities.” This reflects the difference that having consistent, joined-up support can make.
We’re proud of the difference this service is making across our Extra Care schemes.
We’ll continue working closely with partners, listening to feedback and building on what’s working well, so more people can stay independent, feel connected, and live well in their own home.
